How telehealth therapy works in New York
Telehealth is where the weekly work happens. The rule that shapes it is easy to state and easy to get wrong: what matters is which state you are physically sitting in when the session starts.

What telehealth means here
Weekly psychotherapy is telehealth only. Each session is 50 minutes, usually weekly, and booking runs through Carepatron. The format does not change the fee, the cancellation window, or the out-of-network arrangement. It changes where you sit, not what the work is.
The licensing rule, stated plainly
Melanie Van Orden is a Licensed Clinical Social Worker in New York, license 103262-01, and she is licensed in New York only. Licensure for psychotherapy generally follows the client's physical location at the moment of the session, rather than where your mail goes or where the clinician happens to be sitting. So the requirement is that you are physically located in New York State whenever a session takes place. If a trip out of state is coming up, raise it in advance rather than the morning of.

The explanation above is general. Telehealth rules are set by state licensing boards, they are not identical across states, and they change. This page is not legal advice about your situation, and if you split your time between two states or are planning a move, that is a conversation to have directly rather than a question to settle from a web page. What is not in doubt is the practice rule: New York State, at the time of each session.
What you need on your end
A private room matters more than a fast connection, though both help. Headphones keep your half of the conversation yours. Because the work asks you to notice what is going on in your body, sitting somewhere you can actually settle is not a small detail — a parked car between meetings is a compromise, and it is worth naming it as one. If your setup is unreliable, say so early rather than fighting it every week.
What is in person, and what is not
Ketamine-assisted therapy is the exception to telehealth. Those sessions run 180 minutes at $846, in person at Melanie's Manhattan office or in your home. The medical side, meaning prescribing, screening and oversight, belongs to a separate independent licensed medical provider and never to her; she does not prescribe, supply or administer medication. It is not a first-line treatment, and it is offered only alongside ongoing psychotherapy with her.
Coaching sits outside this rule
Coaching is a separate, non-clinical service, and it is offered more widely than New York State. It is not therapy and it is not licensed mental-health treatment. If you are outside New York and what you want is clinical work, coaching is not a version of that. The two are described separately because they are different things.

Questions people ask
Where do I need to be during a session?
Weekly therapy takes place via telehealth. I am licensed in New York and can only offer psychotherapy services to people who reside in New York State. Physically in the state, at the time of each session — that is the standard.
Do you ever meet in person?
Only for ketamine-assisted therapy, which takes place in person at my Manhattan office or in your home. The medication side of it — prescribing, screening and oversight — sits with a separate independent licensed medical provider, never with me. Weekly psychotherapy is telehealth.
Is coaching available outside New York?
Coaching is offered more widely than New York State. It is a separate, non-clinical service, not therapy and not licensed mental-health treatment. Whether it is available where you are is worth asking on the introductory call.
